If you intend to became a member of CMS and you already belong to JČMF, it is enough to fill-in the following form (PostScript, in Czech) (pdf, in Czech) for Czech Mathematical Society and send it to the above address.
In the other case, please contact first the secretary of JČMF.

The annual membership fee is 80 CZK with 50% reducton for students, retired and reciprocal members.
